Commonly Used Forms in the Sasin Ph.D. Program

1. Area Advisor Selection Form: Formally confirms the Area Advisor of Ph.D. study.
2. Ph.D. Progress Report Form: Used to report the progress in the Ph.D. program annually to the Head of the Ph.D. program,
3. Preliminary Program of Study Form: Used to plan for the courses to be taken until graduation.

4. Program of Study Form: Lists all courses required for completion before students can take oral qualifying examinations.
5. Waiver Form: Used to obtain permission to waive a course.
6. Substitution Form: Used to receive approval for substituting one course for another and changing the program sheet.
7. Transfer Form: Used to obtain permission to transfer courses.
8. Application for Individual Investigation or Research Form: Needed to enroll for independent/research study.
9. Outside Minor Approval Form: Required if a student proposes a minor outside Sasin.
10. Application to Take Written Preliminary Qualifying Examinations Form: Should be submitted the quarter before taking the exams.
11. Dissertation Committee Form: Used to obtain formal agreement and approval of the student's selection of Chairperson and other Committee members.
12. Announcement of Dissertation Proposal Defense Form: Used to announce the topic and the date of the Dissertation Proposal Defense.
13. Dissertation Proposal Approval Form: Used to obtain formal approval of the dissertation proposal in order to advance to candidacy.
14. Application for Admission to Candidacy Form: Used to obtain formal approval to advance to candidacy
15. Announcement of the Time and Place of the Ph.D. Final Dissertation Defense Form: Used to announce the topic and the date of the Final Dissertation Defense.
16. Report of Final Dissertation Defense Form: Used to obtain formal approval of the dissertation in order to graduate.
17. Petition to Graduate Form: Used for graduation.

NOTE: The student should submit all of the completed forms to the Head of the Ph.D. Program. Copies of all forms are available from the Ph.D. Office.
